The following documentations must be submitted to the Laboratory for conducting the state quality control of medicines:

   • referral by a territorial authority for quality control of medicines;

   • аct of selection of samples of medicines;

   • samples of medicines with expiration date;

   • standard samples and certificates with expiration dates (in case they are needed for impurities testing, quantitative testing, etc);

   • producer’s quality certificates for each series of medicines samples provided;

   • analytical regulatory documentation and changes to it for the presented samples of medicines;

   • copy of a VAT certificate of the Customer;

   • copy of the EDRPOU certificate (on inclusion of the Customer into the unified state register of Ukraine of enterprises and organizations);

   • bank requisites of the Customer.

   Any testing of samples begins only upon provision of all necessary materials and documents.
